Prius (2010-12) :: Unable To Use EV Mode - Insufficient Charge

Since I purchased my 2010 I've never been able to use EV mode. I know it's just a novelty, and actually hurts gas mileage (due to the need to convert gas to electricity to recharge the battery), so I haven't lost sleep over this, but it is somewhat annoying.
Is there a known problem with 2010s and EV mode? No matter how gently I apply the accelerator it pops out into ECO mode again almost immediately with a message like "insufficient charge" (or something like that), even if the battery is near full charge (I've never seen it fully charged).

It seems like more often than not I am unable to use EV mode and I don't know why. The battery is charged, I'm at a stop! But it says its unavailable.

When I backed up yesterday, some dash symbols showed up. After turning off the car, I am not able to get it in Ready Mode. Only solution so far: to disconnect the 12V battery and wait approx. 2-3 minutes. After that, the lights are off. The 12v battery does also indicates normal voltage. IG-OFF: 12.7V IG-ON 12.2 ... Unfortunately I cannot get the trouble codes with my scan tool ...

I just purchased a '12 PIP advanced a couple days ago. Love it so far, but I'm confused about why it keeps on switching out of EV mode before the charge is used up. My commute is 6 miles, so I assumed that I would be able to get to my office with no problem, using only electric. Without accelerating quickly or going above 62 mph, the car still switches into hybrid mode numerous times, for no apparent reason, resulting in an average of about 90 mpg by the time I get to work, still with a lot of juice left in the battery.
Why this is happening? My dreams of going for months without filling up with gas are starting to fade away.

I was once the proud owner of a 2006 Prius with 106,716 miles on it. Never missed a regularly scheduled service. Now, I am no longer proud, but just another owner of a Prius with a failed transaxle.
Last week, I parked my car on my way to work and, while the car was idling in park, the Triangle of Death (red with exlamation mark within) came up on the dashboard. The car went into limp mode -- it was unable to shift into drive or reverse -- so I had it towed to Darcars in Silver Spring, MD. They thought the original 12v battery was dead, so they replaced it.

I have noticed a mid-pitched humming noise in my '02. I first noticed it about a week after getting four new tires put on it. It is speed dependent, and is most noticeable at about 35 mph.
Somehow I have gotten it in my head that the car might be stuck in "battery charge" mode. I can't tell easily because my car has a permanent power steering problem so I don't have the charge/power car diagram on my dash. I can still feel a difference when I shift into "B" when going downhill.

I don't plug my PiP in often due to the cost of electricity here, but what I have noticed that when it has a full charge, around 13Miles, it will quickly drop down to 7-8, even when not using EV. Meaning, I start up car, turn off EV and the motor gets warm. Engine shuts off and I drive normally using the ICE. Over the course of a few miles, I'd say about 10, that EV range slowly drops down to 7-8 and then maintains itself around there. Pretty much the whole time I would have been using the ICE to maintain ~70MPH on the highway, not climbing, just flat.

I have a concern about my 2010 Prius V(five). I've noticed some issues lately that may all tie in to one big problem.
1. My ev mode will not work at all. My battery will be fully charged and not in "B" mode. Turning car off and on will not work.
2. Engine will not turn off when coming to stops. Mind you when the battery is above 3 lines.
3. Throttle response feels like it's in PWR mode(super sensitive) but the engine is leggy. If I floor it for a sec the RPMS are super slow to come down.
4. The Eco line is quick to go in PWR portion and I'm not pushing it any harder than I was before all of this started. It seems like when I push the gas it skips the electric and automatically pulls from the engine.

Seen the battery charge level at 100%? The battery charge display seems to never go up to the top bar. I think I may have noticed it once, but I might have been dreaming that.

The car has been parked for a few months and i get into it and the navi reads unable to read disk? So I eject it and its perfectly clean. I give it a wipe down just for good measure and slap it back in. after a few minutes and some hesitations it works fine.. Then it happens again like 20 miles away, just goes black and says unable to read disk then comes back on again by itself.. Would a newer updated disk work? I have 90k on it and this is the first time I've ever removed the nav disk, could the lens be dirty..

As I had two extra keys from my last Prius (totaled 2010), I took them to the dealer to get them programmed - they spent gobs of time, but could not get them to register. They verified they were the correct keys - but in the end, could not get through to Toyota's higher level tech support to troubleshoot it....
They said they'd try again on my first service. Why there would be an issue programming extra keys???

Should I be concerned..I have been noticing my battery is charging down overnight. I checked and all lights are off. It does charge immediately in the 10 mile I drive to work..but I am concerned why overnight it should be discharging.
Another Question - The right/left indicator sound is very muted..is that normal? Like, if I have the indicator on to merge into traffic, I am not sure it is as I can barely hear it chirping..

I have a 2010 Prius and have been driving several for the past 7 years.Yesterday, I was driving on the Expressway at 60 mph and all of a sudden I was momentarily stuck at 60 as if I was in Cruise control which I wasn't. Gradually, I was unable to accelerate when I pressed down on the pedal, lost speed and cruised over to the side of the road as if I was out of gas which I wasn't. I shut the engine off, waited a minute and started up again. I continued on my journey without a problem. When I reached my destination I shut down the engine and re started 4 hrs later and drove home 120 miles without a problem. What is the cause of this and is it still under warranty. I have 50,000 miles on the vehicle. May I add, this is the first problem ever encountered.

I have a 2010, Prius III. Whenever I plug my blackberry storm into the usb slot, my phone says "there is not sufficient power to charge the phone". I've tried charging with the Storm 1 and Storm 2 but neither seem to work. I have plugged in my ipod and it seems to charge just fine. Is it just the phone that won't allow it to charge or is it the fact that it's a micro usb that the car may not be able to handle? Or maybe the usb port is faulty.

I've noticed that after sitting over night my battery for the hybrid system is not charged enough to operate the car in EV or for the first mile or two it does not seem to use as much electric power to supplement the car. After a couple miles it is charged or near charged and I see more use.
